Five Whys
Five Whys is a simple technique for attempting to identify the root cause of an issue. It was originally described by Taiichi Ohno for the Toyota Production System.
Instead of trying to answer the question directly, a chain of intermediate causes is uncovered iteratively, until a root cause is found, by repeatedly asking why something happened.
Five is only a rough number of steps that usually lead to a reasonable root cause (i.e. one that can be acted upon or controlled). In practice it may take more or less steps depending on the particular issue and the level of understanding of the causal forces at play.
